Reconstructive Pelvic Surgeries

Reconstructive pelvic surgeries encompass procedures such as hysterectomy, salpingo-oophorectomy, complete vaginectomy, orchiectomy, clitoroplasty, and urethroplasty. These surgeries may be necessary due to congenital conditions like vaginal agenesis and vaginal septum or acquired issues such as radiation complications and labial hypertrophy.

Hysterectomies

A hysterectomy involves removing a woman’s uterus, potentially along with the fallopian tubes and ovaries, resulting in the inability to become pregnant and cessation of menstrual periods. There is more than one type of hysterectomy.

1. Total Hysterectomy: Removal of the entire uterus and cervix, with possible removal of the ovaries and fallopian tubes.

2. Partial, Subtotal, or Supracervical Hysterectomy: Removal of the upper part of the uterus, with optional removal of the ovaries.

3. Radical Hysterectomy: Removal of the cervix, uterus, tissues around the cervix, and the upper part of the vagina, with possible removal of the ovaries and fallopian tubes.

Salpingo-Oophorectomy

This procedure involves the removal of the ovaries and fallopian tubes and can be performed laparoscopically, through a large abdominal or pelvic incision, or robotically.

Complete Vaginectomy

This procedure entails the removal of the vagina under general anesthesia, with incisions made to separate the vaginal tube from surrounding tissues.

Orchiectomy

An orchiectomy involves removing one or both testicles, with an incision made in the pubic area or scrotum. The procedure takes 30 to 60 minutes and may include removal of the spermatic cord.

Clitoroplasty

A clitoroplasty is performed to create a normal-appearing clitoris that retains sensation. It can be done through clitoral recession or reduction clitoroplasty.

Urethroplasty

This procedure repairs or replaces the urethra by removing scar tissue and connecting the healthy ends or using tissue from other parts of the body, such as the mouth, genitals, or rectum, to rebuild it.

Surgeries for Other Conditions

Patients with congenital anomalies such as vaginal agenesis may undergo vaginoplasty, which can be performed using tissue grafts, a medical traction device, or a portion of the colon. Those with a vaginal septum may require resection surgery, while labial hypertrophy may be treated with a labiaplasty to remove excess skin and reshape the labia.
